Making Your Appointment
To set up an appointment for an abortion, call Women’s Health Horizons between 9:00 am and 4:30 pm Monday through Friday.
We can usually schedule you within two to three days of your call. Occassionally, same day appointments are available.
We will need to know when the first day of your last menstrual period was. You are welcome to call us with any questions you may have prior to actually scheduling your appointment.
We participate with most insurance plans with prior verification and proper referrals. If you have insurance please tell us when you call for your appointment. We do not accept Medicaid.
After you have scheduled your appointment, you can download the form below that you will need for your appointment. If you have access to a computer, download your patient forms, fill them out, and bring them with you to your scheduled appointment -- saving you time on the day of your appointment in our office.

- You will also be asked not to eat or drink 4 hours before your appointment.
- If you take medication on a regular basis, you may take your medication that day.
- It is always a good idea to wear loose fitting comfortable clothes the day of your appointment.
- It is always recommended that you bring one person with you the day of your appointment, but not necessary.
- However, if you are planning to have the OPTIONAL, but recommended conscious sedation, it is ABSOLUTELY NECESSARY, that you have someone with you to drive you home after your appointment!
- It is also a good idea to bring a sweatshirt and socks, as some people who are waiting in the waiting room for some period of time, start to feel very cold.
